When your doctor determines that you no longer need the specialized services of our hospital but still require medical care, he or she may discharge you to a skilled nursing facility, a rehabilitation center, or to home care. Our discharge planner will help arrange for the services you may need after leaving the hospital.Medicare and supplemental insurance policies have limited coverage for skilled nursing care and home healthcare. You should check your policies to be certain what services will be covered. Social services can assist you with information regarding guidelines for covered skilled care services.
